In accordance with IRS requirements, Berryville Schools will furnish Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, for tax year 2025 to eligible employees and former employees upon request.
Form 1095-C provides information about the health coverage offered to you during 2025.
